Observation

After interviewing parties involved in the incident, it was determined that active supervision was not provided while transporting children in the program van. Staff person Maxwell Stelzner was the only staff person in the vehicle with 10 children. It was determined that one child forced a second child's face into their crotch area. The staff person was unaware of this incident until the van trip was completed.
